This combination therapy includes three agents: atezolizumab (a PD-L1 immune checkpoint inhibitor monoclonal antibody), bevacizumab (a monoclonal antibody targeting vascular endothelial growth factor A, or VEGF-A), and TPST-1120 (also known as amezalpat, a first-in-class oral small molecule antagonist of peroxisome proliferator-activated receptor alpha, or PPARα). The combination is under investigation for use as first-line therapy in patients with unresectable or metastatic hepatocellular carcinoma (HCC). TPST-1120 aims to inhibit tumor metabolism and immune suppression via PPARα antagonism, while atezolizumab blocks immune escape by targeting PD-L1, and bevacizumab inhibits tumor angiogenesis by targeting VEGF-A. This triplet regimen is being evaluated in randomized, controlled phase 3 clinical trials and has shown improved survival and response rates over standard-of-care atezolizumab plus bevacizumab in early-phase studies[2][3][6][7].
