Atezolizumab plus cisplatin and 5-fluorouracil is a combination therapy investigated for the treatment of advanced squamous cell carcinoma of the anus (anal cancer). The regimen combines the immune checkpoint inhibitor **atezolizumab** (Tecentriq) with a chemotherapy backbone of **cisplatin** and **5-fluorouracil** (often as part of a modified docetaxel, cisplatin, and fluorouracil, or mDCF, regimen). Atezolizumab is a humanized monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and B7.1 receptors to restore anti-tumor T-cell activity. Cisplatin is a platinum-based cytotoxic agent that induces DNA cross-linking, while 5-fluorouracil is an antimetabolite that inhibits thymidylate synthase, thereby disrupting DNA synthesis. This combination was notably evaluated in the Phase 2 SCARCE (PRODIGE 60) trial, which assessed its efficacy as a first-line treatment for advanced anal squamous cell carcinoma.
