Overview

**Atezolizumab + daratumumab + lenalidomide** is an investigational triple combination therapy comprising three distinct drugs: atezolizumab (a programmed death-ligand 1 [PD-L1] inhibitor, immune checkpoint inhibitor), daratumumab (a monoclonal antibody targeting CD38), and lenalidomide (an immunomodulatory agent). - **Atezolizumab** enhances antitumor immunity by blocking PD-L1, thereby preventing cancer cells from evading immune detection and destruction. - **Daratumumab** binds CD38 on plasma cells (including malignant cells in multiple myeloma), promoting antibody-dependent cellular cytotoxicity and other immune-mediated mechanisms. - **Lenalidomide** modulates the immune response, exhibiting anti-angiogenic and direct antineoplastic activity, and is commonly used in hematologic malignancies. This specific triple regimen has not entered routine clinical use or gained regulatory approval as of current evidence. While each drug (and some doublet combinations, such as daratumumab + lenalidomide or atezolizumab + daratumumab) has been studied in clinical trials (notably for multiple myeloma and solid tumors), no published clinical trials currently support the combined use of all three agents together as a distinct regimen. The scientific rationale for combining these agents would be rooted in enhancing immune-mediated anti-tumor activity and direct tumor cell killing.
