EC/EP + atezolizumab is a combination chemo-immunotherapy regimen used as a first-line treatment for extensive-stage small cell lung cancer (ES-SCLC). The regimen consists of the PD-L1 inhibitor atezolizumab (Tecentriq) administered alongside etoposide and a platinum-based chemotherapy agent, which can be either carboplatin (EC) or cisplatin (EP). Atezolizumab is a humanized monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and B7.1 receptors to restore anti-tumor T-cell activity. Etoposide is a topoisomerase II inhibitor that causes DNA strand breaks, while carboplatin and cisplatin are platinum-based alkylating agents that form DNA cross-links, leading to cell death. This combination therapy was established as a standard of care following the IMpower133 trial, which demonstrated improved overall survival and progression-free survival compared to chemotherapy alone.
