Overview

This is a four-drug combination regimen consisting of atezolizumab, etoposide, carboplatin, and tarlatamab. Atezolizumab is a monoclonal antibody that inhibits programmed death-ligand 1 (PD-L1), thereby enhancing T-cell–mediated immune responses against tumor cells. Etoposide is a topoisomerase II inhibitor that induces DNA strand breaks and apoptosis in rapidly dividing cells. Carboplatin is a platinum-based chemotherapeutic agent that causes DNA crosslinking and cell death. Tarlatamab is a bispecific T-cell engager targeting delta-like ligand 3 (DLL3) on small cell lung cancer cells and CD3 on T-cells, redirecting cytotoxic T-cells to kill DLL3-expressing tumor cells. This combination leverages immunotherapy with checkpoint inhibition (atezolizumab), cytotoxic chemotherapy (etoposide/carboplatin), and targeted immunotherapy via bispecific engagement of DLL3-positive tumor cells by tarlatamab. The primary indication for the three-drug backbone of atezolizumab + etoposide + carboplatin is extensive-stage small cell lung cancer; the addition of tarlatamab represents an investigational approach aiming to further improve outcomes in this setting[1][5][10].
