Atezolizumab + vemurafenib is a combination therapy for BRAF V600 mutation-positive unresectable or metastatic melanoma. Atezolizumab is a humanized IgG1 monoclonal antibody targeting programmed cell death ligand 1 (PD-L1), blocking its interaction with both PD-1 and B7.1 receptors, thereby promoting T cell-mediated antitumor immunity. Vemurafenib is an orally administered, small molecule inhibitor of the mutated BRAF (mostly V600E and V600K), a serine/threonine kinase involved in MAPK pathway signaling, where it blocks tumor proliferation driven by aberrant MAPK pathway activation. The dual blockade of MAPK signaling and the PD-1/PD-L1 immunosuppressive axis synergistically improves tumor immune recognition and growth suppression. This dual combination is usually approved and studied as part of a triplet regimen with cobimetinib (a MEK inhibitor), but the combination of atezolizumab plus vemurafenib represents the core immunotherapy plus targeted therapy approach for advanced melanoma with BRAF mutations[1][2][3][8][10].
