Overview

ATLCAR.kappa.28 is an autologous chimeric antigen receptor (CAR) T-cell therapy developed for the treatment of B-cell malignancies that express the immunoglobulin kappa (κ) light chain. This second-generation CAR construct consists of a single-chain variable fragment (scFv) derived from a κ-specific monoclonal antibody, which is fused to a CD28 costimulatory domain and a CD3ζ signaling domain. Unlike pan-B-cell therapies targeting CD19, ATLCAR.kappa.28 specifically targets the light chain restriction of the malignant clone. This approach is designed to eliminate κ-positive neoplastic B-cells while sparing normal λ-positive B-cells, potentially mitigating the severity of B-cell aplasia and associated hypogammaglobulinemia. The therapy is currently being evaluated in clinical trials for patients with relapsed or refractory B-cell Non-Hodgkin Lymphoma, Chronic Lymphocytic Leukemia, and Multiple Myeloma.
