Atoguanil is a novel combination drug comprising atovaquone (ATV) and proguanil (PG), designed to enhance the bioavailability of atovaquone compared to the existing antimalarial drug Malarone®. It is being developed as an antimalarial treatment. Atovaquone primarily acts by selectively inhibiting the parasitic mitochondrial cytochrome bc1 complex, disrupting the electron transport chain and indirectly inhibiting pyrimidine biosynthesis. Proguanil, through its active metabolite cycloguanil, inhibits parasitic dihydrofolate reductase, thereby interfering with folate synthesis and DNA replication in the parasite. This dual mechanism provides synergistic antimalarial activity, aiming for improved efficacy and reduced potential for resistance development.
