Atorvastatin + mesalamine is a combination of two small molecule drugs used in clinical research for the treatment of ulcerative colitis (UC). Atorvastatin is an HMG-CoA reductase inhibitor (statin) primarily used to lower cholesterol, but it also exhibits anti-inflammatory and antioxidant properties. Mesalamine (also known as 5-aminosalicylic acid or 5-ASA) is an aminosalicylate anti-inflammatory agent commonly used to treat mild to moderate UC. The combination has been shown in clinical studies to reduce disease activity, inflammatory biomarkers (such as calprotectin, CRP, ESR), oxidative stress markers, and improve mucosal healing compared with mesalamine alone. Mechanistically, the benefits are attributed to inhibition of pro-inflammatory pathways including NF-κB signaling by both agents; activation of adenosine monophosphate-activated protein kinase (AMPK) by mesalamine; reduction in oxidative stress via increased antioxidant enzyme production by atorvastatin; and modulation of cytokines such as IL-6 and TNF-alpha[1][3][9]. This combination is being investigated as adjunctive therapy for patients with UC who may not achieve full remission on standard therapy.
