This therapeutic combination, being investigated by CHU de Quebec-Universite Laval, involves the simultaneous administration of three cholesterol-lowering agents—atorvastatin, ezetimibe, and evolocumab—in conjunction with standard-of-care FOLFIRINOX chemotherapy. The regimen is specifically designed for patients with advanced or metastatic pancreatic ductal adenocarcinoma (PDAC). The biological rationale is based on the high cholesterol requirements of pancreatic cancer cells for growth, membrane synthesis, and metastasis. By inhibiting HMG-CoA reductase (atorvastatin), blocking intestinal cholesterol absorption via NPC1L1 (ezetimibe), and reducing LDL receptor degradation through PCSK9 inhibition (evolocumab), the combination aims to induce a profound systemic and cellular cholesterol shortage. This metabolic reprogramming is intended to starve the tumor cells, inhibit their progression, and potentially sensitize them to the cytotoxic effects of chemotherapy.
