Atorvastatin + folic acid is an investigational drug combination being evaluated for its potential to stabilize carotid atherosclerotic plaques. Atorvastatin is an HMG-CoA reductase inhibitor (statin) that lowers LDL cholesterol and exerts pleiotropic cardiovascular effects, including anti-inflammatory and plaque-stabilizing properties. Folic acid is a B-vitamin that serves as a cofactor in the folate/homocysteine metabolic pathway, effectively reducing serum homocysteine levels, which are associated with increased vascular risk. The combination aims to provide incremental benefit in stabilizing vulnerable carotid plaques compared to statin therapy alone by addressing both lipid-mediated and homocysteine-mediated pathways of atherosclerosis. It is currently being studied in the PROMISE trial by Ruijin Hospital.
