Overview

Atreleuton is a synthetic, orally bioavailable small molecule that acts as a potent, selective, and reversible inhibitor of the enzyme 5-lipoxygenase (5‑LO). By competitively binding to the active site of 5-lipoxygenase, it blocks the conversion of arachidonic acid to leukotrienes—proinflammatory lipid mediators implicated in cardiovascular inflammation and plaque instability. This mechanism reduces leukotriene production (including LTB4), aiming to mitigate inflammatory processes involved in diseases such as atherosclerosis and coronary artery disease. Clinical studies have shown that atreleuton can decrease epicardial and pericardial adipose tissue volumes and may favorably alter plaque volume and composition in patients with recent acute coronary syndromes[2][3][4][5][6][7]. The drug has reached up to phase II clinical trials for these indications.
