Overview

**Atropine + obidoxime** is an injectable combination drug used primarily as an antidote for poisoning by organophosphate nerve agents and pesticides. Atropine is a competitive antagonist (inhibitor) at muscarinic acetylcholine receptors, providing symptomatic relief by blocking excessive muscarinic activity (e.g., bronchorrhea, bradycardia, secretions). Obidoxime is an oxime that reactivates acetylcholinesterase (AChE) by removing the phosphate group from the enzyme's active site, thereby reversing the underlying cause of toxicity and alleviating both muscarinic and certain nicotinic effects (e.g., muscle weakness) of organophosphate poisoning. The combination is formulated for rapid intramuscular injection, often in autoinjectors for emergency use in chemical warfare or pesticide poisoning scenarios. The combination's efficacy addresses both muscarinic and nicotinic manifestations of poisoning, providing synergistic benefit over monotherapy[1][3][10].
