Atropine + pralidoxime + magnesium sulfate is an investigational combination regimen being studied for the treatment of acute organophosphate (OP) poisoning. This therapeutic approach combines three agents with distinct mechanisms to address the complex pathology of OP toxicity: atropine serves as a muscarinic receptor antagonist to block the effects of excess acetylcholine; pralidoxime acts as an acetylcholinesterase reactivator to restore the function of the inhibited enzyme; and magnesium sulfate is used as an adjunctive agent to stabilize neuromuscular junctions and provide calcium channel blockade. The regimen is primarily being evaluated in clinical trials to determine if the addition of magnesium sulfate to standard atropine and pralidoxime therapy can reduce mortality and the total requirement for atropine in poisoned patients.
