Overview

The combination of atropine and succinylcholine is used in medical procedures, particularly during rapid sequence intubation. Atropine is an anticholinergic medication administered before succinylcholine to prevent bradycardia (slow heart rate), a potential side effect of succinylcholine. Succinylcholine is a depolarizing neuromuscular blocker used for tracheal intubation and skeletal muscle relaxation during surgery or mechanical ventilation. It acts by binding to cholinergic receptors at the motor endplate, causing depolarization and muscle paralysis. Historically recommended as premedication before succinylcholine, especially in pediatric patients, recent evidence suggests this may be unnecessary for single-dose use, particularly in emergency settings. Some research indicates atropine premedication may increase ventricular dysrhythmias in certain situations. The practice of routine atropine use before succinylcholine varies among physicians. Atropine can be administered intravenously or intramuscularly before succinylcholine. IV atropine can significantly increase heart rate within 10 minutes, while IM administration may not have the same immediate effect. Currently, routine atropine premedication for succinylcholine is not widely recommended due to lack of supporting evidence, but it may be considered when there is an increased risk of bradycardia, such as during intubation.
