Autologous adipose-derived mesenchymal stem cells (ADSCs) are an investigational cell therapy being evaluated by Nantes University Hospital for the treatment of complex anal fistulas in patients with Crohn's disease. This therapy involves the extraction of a patient's own adipose tissue via lipoaspiration, followed by the isolation of the mesenchymal stem cell fraction, which is then injected locally into the tissues surrounding the fistula tract. ADSCs are characterized by their potent immunomodulatory, anti-inflammatory, and regenerative properties. They are thought to facilitate the closure of difficult-to-treat fistulas by reducing local inflammation and promoting tissue repair, serving as a potentially more accessible and cost-effective alternative to allogeneic stem cell products like darvadstrocel (Alofisel).
