Autologous adipose-derived stem cells (ADSCs) are a type of mesenchymal stem cell isolated from a patient's own adipose tissue. In the clinical program led by the Arkansas Heart Hospital, these cells are administered as part of a minimally manipulated stromal vascular fraction (SVF) for the treatment of moderate to severe lower extremity peripheral arterial disease (PAD). The therapeutic approach utilizes a multi-route delivery strategy, including intravenous, intra-adventitial, and intramuscular injections, to maximize the distribution of cells to ischemic tissues. The mechanism of action is primarily paracrine, where the cells secrete various pro-angiogenic and immunomodulatory factors that stimulate the formation of new blood vessels (angiogenesis), reduce local inflammation, and promote tissue regeneration in limbs that are otherwise non-revascularizable. The hospital is also investigating the use of low-frequency ultrasound (LoFU) as an adjunct to enhance the homing and therapeutic efficacy of the administered cells.
