Autologous adipose-derived stem cell therapy is an investigational cell-based treatment being developed by the Centre Hospitalier Regional De Marseille (AP-HM) for the management of complex perianal fistulas in patients with Crohn's disease. The therapy involves harvesting mesenchymal stem cells (MSCs) from the patient's own adipose tissue via liposuction, followed by processing and local intralesional injection into the fistula tracts. These cells exert therapeutic effects through immunomodulation and the secretion of paracrine factors, including pro-angiogenic and anti-inflammatory cytokines. By suppressing local inflammation and promoting tissue regeneration, the therapy aims to achieve fistula closure in patients who are refractory to conventional medical and surgical interventions. Clinical evaluation is primarily conducted through the ADIPOCROHN trial series.
