Autologous bone marrow aspirate (BMA) is a biologic therapy derived from a patient's own bone marrow, typically harvested from the posterosuperior iliac crest. It contains a heterogeneous population of cells, including mesenchymal stem cells (MSCs), hematopoietic stem cells, and white blood cells, as well as a concentrated pool of regenerative growth factors such as platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and bone morphogenetic proteins (BMP-2 and BMP-7). In the context of chronic plantar fasciitis, BMA is investigated for its potential to promote tissue healing and modulate inflammation in recalcitrant cases. The Federal University of São Paulo (UNIFESP) is conducting a randomized, double-blind clinical trial to evaluate the efficacy of ultrasound-guided BMA injections compared to platelet-rich fibrin (PRF) for this indication.
