Overview

Autologous bone marrow cell concentrate, commonly referred to as BMAC, is a biologic therapy obtained by aspirating bone marrow from the patient's iliac crest or other bone. The aspirate is then processed—most often by centrifugation—to concentrate the nucleated cell fraction, including mesenchymal stem cells, hematopoietic stem cells, platelets, growth factors, and cytokines. BMAC is injected back into injured or degenerative tissues to promote healing, regeneration, and modulation of inflammation. Its mechanism is believed to include both direct tissue regeneration by stem cell differentiation and paracrine signaling through growth factors and cytokines[1][4][7][9]. BMAC is not manipulated beyond concentration and is not cultured or expanded, which keeps it within FDA regulatory guidelines for minimal manipulation[1][3][4][9]. Indications include osteoarthritis, cartilage injury, tendon injury, bone fractures, osteonecrosis, and wound healing[2][4][7].
