Autologous bone marrow-derived adult mononuclear cells (BM-MNCs) are a heterogeneous population of cells harvested from a patient's own bone marrow, typically via iliac crest aspiration. This cell fraction includes hematopoietic stem cells, mesenchymal stem cells, and endothelial progenitor cells. Developed by the Andalusian Network for Design and Translation of Advanced Therapies (Red Andaluza de Diseño y Traslación de Terapias Avanzadas), this cell therapy is primarily investigated for the treatment of idiopathic dilated cardiomyopathy (DCM). The therapeutic mechanism is predominantly paracrine; once administered (often via intracoronary injection), the cells secrete various growth factors and cytokines, such as VEGF and FGF, which promote angiogenesis, inhibit cardiomyocyte apoptosis, and modulate the local inflammatory environment to facilitate myocardial repair and improve cardiac function. Clinical trials, such as the CELL-DCM study, have evaluated the safety and efficacy of these cells in improving left ventricular ejection fraction and clinical outcomes in heart failure patients.
