This autologous bone marrow-derived mesenchymal stem cell (BMMSC) therapy is an Advanced Therapy Medicinal Product (ATMP) developed by University College London for the treatment of chronic midportion Achilles tendinopathy. The therapy involves the extraction of a patient's own bone marrow, followed by the culture-expansion of mesenchymal stem cells, which are then injected directly into the damaged Achilles tendon. The mechanism of action relies on the stem cells' capacity for self-renewal and differentiation into tenocytes, as well as their ability to secrete paracrine factors that modulate inflammation and promote tissue repair. A Phase IIa proof-of-concept trial (ASCAT) demonstrated the safety and potential efficacy of this approach in improving pain and functional scores in patients with chronic tendinopathy.
