Autologous bone marrow-derived mononuclear cells (BMMNCs) represent a heterogeneous cell population harvested from a patient's own bone marrow, typically via iliac crest aspiration. This mixture includes hematopoietic stem cells, mesenchymal stem cells, endothelial progenitor cells, and various mature leukocytes. The therapeutic rationale relies on the paracrine effect, where the transplanted cells secrete a cocktail of neurotrophic factors (such as NGF and GDNF), angiogenic factors (like VEGF and FGF2), and anti-inflammatory cytokines to modulate the local microenvironment, reduce apoptosis, and stimulate endogenous repair. Because the cells are autologous, the risk of immune rejection is minimized. BMMNCs are currently being investigated across a broad spectrum of indications, particularly neurological disorders like cerebral palsy, stroke, and spinal cord injury, as well as cardiovascular and ischemic limb diseases.
