Autologous bone marrow-derived mononuclear cells (BM-MNCs) are a cell therapy product consisting of a concentrated fraction of mononuclear cells harvested from a patient's own bone marrow. Developed by the Leiden University Medical Center (LUMC), this therapy is specifically investigated for patients with peripheral artery disease (PAD), including those with severe claudication or critical limb ischemia (CLI). The cells are typically administered via multiple intramuscular injections into the affected limb's muscle (e.g., calf muscle). The therapeutic rationale is based on therapeutic arteriogenesis, where the transplanted cells—which include hematopoietic stem cells, mesenchymal stem cells, and endothelial progenitor cells—release a cocktail of pro-angiogenic growth factors and cytokines. These factors stimulate the growth of new capillaries (angiogenesis) and the enlargement of pre-existing collateral vessels (arteriogenesis), ultimately aiming to restore blood perfusion and prevent amputation.
