Autologous bone marrow-derived mononuclear cells (BM-MNCs) are a heterogeneous population of cells, including hematopoietic stem cells, mesenchymal stem cells, and endothelial progenitor cells, harvested from a patient's own bone marrow. Developed by the Stem Cell and Regenerative Medicine (SCARM) Institute in Tabriz, Iran, this cell therapy is investigated for the treatment of heart failure with reduced left ventricular ejection fraction (LVEF <=30%). The cells are delivered either via intracoronary infusion or direct intramyocardial injection during coronary artery bypass grafting (CABG). The therapeutic mechanism is primarily attributed to paracrine effects, where the injected cells secrete growth factors and cytokines that promote angiogenesis, reduce apoptosis, and modulate inflammation, thereby potentially improving myocardial function and remodeling.
