Overview

Autologous bone marrow-derived progenitor cells are a form of cell therapy in which multipotent or lineage-committed progenitor (stem) cells are harvested from a patient's own bone marrow and then reintroduced into the same patient. These therapies may use either hematopoietic stem/progenitor cells (which give rise to blood and immune system lineages) or mesenchymal stromal/stem cells (BM-MSCs), which can differentiate into various connective tissue types. The mechanism of action is primarily regenerative—these progenitor/stem cells home to sites of injury or disease and contribute to tissue repair by differentiating into needed cell types and/or secreting trophic factors that modulate inflammation and promote healing. This approach avoids immunological incompatibility since the donor and recipient are the same individual. Autologous bone marrow-derived progenitor cell therapy has been investigated for a range of indications including hematologic malignancies (as part of autologous stem cell transplantation following high-dose chemotherapy), acute myocardial infarction, critical limb ischemia, amyotrophic lateral sclerosis (ALS), reproductive disorders such as Asherman's syndrome, atrophic endometrium, recurrent implantation failure, as well as other degenerative conditions[1][2][6][8][10]. The specific subpopulation used may vary by indication; for example, BM-MSCs have been conditionally approved in South Korea for ALS under the product name lenzumestrocel[10].
