Autologous bone marrow nucleated cells (BMNCs) are a heterogeneous population of cells harvested from a patient's own bone marrow, typically via aspiration from the iliac crest. This cell population includes hematopoietic stem cells, mesenchymal stem cells, endothelial progenitor cells, and various mature leukocytes. The therapeutic application involves concentrating these cells—often using specialized centrifugation systems like the Harvest BMAC or Terumo SmartPRep2—and delivering them to damaged tissues. The mechanism of action is primarily attributed to paracrine effects, where the cells secrete a variety of cytokines, chemokines, and growth factors (such as VEGF and BDNF) that promote angiogenesis, reduce inflammation, and support endogenous tissue repair. BMNCs are being investigated for a wide range of conditions, including cerebral palsy, congestive heart failure, ischemic stroke, and spinal cord injury, with delivery routes ranging from intrathecal and intravenous to direct intramyocardial injection.
