Overview

Autologous CAR-T cell injection targeting EpCAM is a cellular immunotherapy consisting of a patient's own T cells genetically engineered to express a chimeric antigen receptor (CAR) specific for the Epithelial Cell Adhesion Molecule (EpCAM). EpCAM is a transmembrane glycoprotein that is highly overexpressed in various epithelial carcinomas, including gastrointestinal, breast, and lung cancers, while maintaining limited expression in normal tissues. A specific example of this therapy is **IMC001**, developed by Immunofoco, which is currently undergoing Phase I/IIa clinical evaluation (e.g., NCT05028933) for patients with advanced epithelial solid tumors. The therapy is designed to recognize and bind EpCAM on the surface of tumor cells, triggering T-cell activation, cytokine release, and targeted lysis of the malignant cells. Clinical monitoring for these therapies often focuses on potential on-target, off-tumor toxicities, particularly in the gastrointestinal tract where low levels of EpCAM are expressed.
