Overview

Autologous CD34+ cell therapy is an investigational cell-based biologic developed by the Mayo Clinic for the treatment of coronary endothelial dysfunction (CED) and early coronary atherosclerosis, particularly in patients with nonobstructive coronary artery disease (NOCAD). The therapy utilizes the patient's own CD34+ hematopoietic stem and progenitor cells, which are known to play a vital role in vascular repair and microcirculatory maintenance. In patients with CED, these cells are often reduced in number or function. The treatment process involves mobilizing CD34+ cells from the bone marrow into the peripheral blood using granulocyte colony-stimulating factor (G-CSF), followed by collection via leukapheresis. The isolated CD34+ cells are then delivered via a single intracoronary infusion into the left anterior descending artery. The mechanism of action involves the cells' ability to promote angiogenesis, reduce inflammation through paracrine effects, and restore endothelial integrity, thereby potentially modifying the course of microvascular disease.
