Overview

Autologous CD34+ peripheral blood stem cells refer to a cell therapy product used in hematopoietic stem cell transplantation (HSCT) for autoimmune diseases like systemic sclerosis. The process involves mobilizing hematopoietic stem cells from the patient's bone marrow into the peripheral blood, typically using G-CSF and cyclophosphamide, followed by collection via leukapheresis. The collected cells undergo CD34+ selection to enrich for stem cells while depleting T-cells, which are thought to drive the underlying autoimmune pathology. Following a conditioning regimen of high-dose chemotherapy and total body irradiation to ablate the patient's dysfunctional immune system, the CD34+ selected cells are re-infused to reconstitute a healthy, non-autoreactive immune system. This specific protocol was evaluated by Baylor College of Medicine in a Phase 1 clinical trial (NCT00114530) for patients with severe systemic sclerosis.
