Overview

Autologous CD34+ stem cells (University Medical Centre Ljubljana) is an investigational cell-based biologic therapy being developed for the treatment of nonischemic dilated cardiomyopathy (DCM). The therapy involves mobilizing CD34+ hematopoietic progenitor cells from the patient's bone marrow using granulocyte colony-stimulating factor (G-CSF), collecting them via apheresis, and subsequently administering them to the patient through intracoronary infusion or transendocardial injection into areas of reduced myocardial viability. The biological mechanism of action is centered on neoangiogenesis; the transplanted CD34+ cells facilitate the formation of new blood vessels within the myocardium by either directly integrating into the vascular endothelium or by secreting paracrine factors, such as angiogenic cytokines, that promote tissue repair and improve myocardial perfusion. Clinical studies, including several Phase 2 trials conducted by the University Medical Centre Ljubljana, have demonstrated improvements in left ventricular ejection fraction and exercise capacity in patients with end-stage heart failure.
