Overview

Autologous cold-stored platelets are a cellular therapy product derived from a patient's own blood and stored at refrigerated temperatures (typically 1°C to 6°C). Unlike standard room-temperature platelets (20°C to 24°C), cold storage preserves platelets in a "primed" or pre-activated state. This storage condition helps maintain high levels of surface receptors, such as the glycoprotein Ib-IX-V complex, and promotes the exposure of procoagulant phospholipids like phosphatidylserine on the cell surface. These changes result in enhanced adhesion to damaged vascular endothelium and accelerated thrombin generation upon re-infusion. The primary therapeutic goal is to provide rapid hemostasis in perioperative settings, particularly in cardiac surgery, while eliminating the risks associated with allogeneic transfusions, such as transfusion-transmitted infections and transfusion-related acute lung injury (TRALI).
