Autologous concentrated bone marrow, commonly referred to as bone marrow aspirate concentrate (BMAC), is a cell-based therapy produced by centrifuging a patient's own bone marrow aspirate at the point of care. This process concentrates the mononuclear cell fraction, which includes a heterogeneous population of mesenchymal stem cells (MSCs), hematopoietic stem cells (HSCs), and endothelial progenitor cells, alongside a high concentration of regenerative growth factors such as TGF-β, PDGF, and VEGF. In the treatment of idiopathic osteonecrosis of the femoral head (ION), the concentrate is typically injected into the necrotic area of the bone, often following core decompression. The therapeutic mechanism involves the stimulation of osteogenesis and angiogenesis, promoting bone repair and revascularization to prevent the structural collapse of the femoral head.
