Autologous concentrated bone marrow cells, frequently referred to as bone marrow aspirate concentrate (BMAC), is a regenerative cell therapy product derived from a patient's own bone marrow. The preparation involves aspirating bone marrow (typically from the iliac crest) and concentrating the mononuclear cell fraction through centrifugation. This concentrate contains a heterogeneous population of cells, including mesenchymal stem cells (MSCs), hematopoietic stem cells, and endothelial progenitor cells, as well as a high concentration of cytokines and growth factors such as VEGF, BMP-2, and TGF-beta. In the context of idiopathic osteonecrosis of the femoral head, these cells are transplanted directly into the necrotic bone lesion. The therapeutic rationale is that the transplanted cells and growth factors will stimulate angiogenesis and osteogenesis, promoting the repair of necrotic bone and potentially delaying or preventing the collapse of the femoral head and the subsequent need for total hip arthroplasty.
