Autologous dendritic cells expressing CMV pp65 is an investigational cellular vaccine developed by Antwerp University Hospital for the prophylaxis of cytomegalovirus (CMV) infection in kidney transplant candidates. The therapy involves harvesting a patient's own monocytes, differentiating them into dendritic cells ex vivo, and transfecting them with messenger RNA (mRNA) encoding the immunodominant CMV pp65 tegument protein (encoded by the UL83 gene). Upon intradermal administration, these modified dendritic cells present pp65-derived peptides to the immune system, priming and expanding CMV-specific CD4+ and CD8+ T-cell responses. This approach aims to establish protective immunity in CMV-seronegative patients prior to transplantation, potentially reducing post-transplant viral reactivation and the need for antiviral therapy.
