This personalized neoantigen-pulsed autologous dendritic cell (nDC) vaccine is an investigational immunotherapy developed by Beijing Tiantan Hospital for the treatment of newly diagnosed glioblastoma multiforme (GBM). The manufacturing process involves identifying patient-specific tumor neoantigens through whole-exome and RNA sequencing of the tumor tissue. Autologous dendritic cells are then derived from the patient's peripheral blood mononuclear cells and pulsed with multiple (typically 3-20) personalized neoantigen peptides. Once administered, these dendritic cells present the neoantigens to the host immune system, specifically activating CD4+ and CD8+ T cells to recognize and eliminate glioblastoma cells harboring those specific mutations. The vaccine is currently being evaluated in Phase I clinical trials (e.g., NCT04968366) in combination with standard-of-care temozolomide chemotherapy.
