Autologous hematopoietic stem cell transplantation (HSCT) for Crohn's disease is a therapeutic procedure pioneered by Dr. Richard Burt at Northwestern University. The protocol is designed for patients with severe, treatment-refractory Crohn's disease who have failed conventional therapies, including biological agents. The procedure involves three main stages: mobilization of peripheral blood stem cells using cyclophosphamide and granulocyte colony-stimulating factor (G-CSF), conditioning with a non-myeloablative regimen of high-dose cyclophosphamide and anti-thymocyte globulin (ATG) to achieve immune ablation, and the subsequent reinfusion of the patient's own CD34-enriched stem cells. This approach aims to reset the immune system by eliminating autoreactive lymphocytes and allowing for the regeneration of a self-tolerant immune repertoire, potentially leading to long-term drug-free remission.
