Autologous hematopoietic stem cell transplantation (AHSCT) is an intensive cellular therapy protocol developed and investigated by Northwestern University for patients with refractory idiopathic inflammatory myopathy (IIM). The procedure involves the mobilization and collection of the patient's own hematopoietic stem cells, followed by a potent conditioning regimen designed to ablate the existing autoreactive immune system. This regimen typically includes high-dose cyclophosphamide, anti-thymocyte globulin (ATG), and rituximab. Following immunoablation, the harvested stem cells are reinfused into the patient to reconstitute a new, self-tolerant immune system. This "immune reset" aims to provide long-term disease remission in patients who have failed conventional immunosuppressive therapies.
