This personalized autologous dendritic cell vaccine is an investigational immunotherapy developed by the Federal University of São Paulo (UNIFESP) in collaboration with the São Paulo AIDS Research Center (SPARC) for the treatment and potential eradication of HIV-1. The vaccine is produced by isolating a patient's monocytes, differentiating them into dendritic cells ex vivo, and pulsing them with a cocktail of HIV-1 antigens, specifically the Gag, Vpr, Vif, and Nef proteins. Upon re-administration, these dendritic cells act as potent antigen-presenting cells to prime and expand HIV-specific CD4+ and CD8+ T-cell responses. The vaccine is currently being evaluated in the SPARC-7 clinical trial as part of a multi-pronged 'shock and kill' strategy that includes antiretroviral therapy intensification (Maraviroc and Dolutegravir) and the use of latency-reversing or purging agents (nicotinamide and auranofin) to target the latent HIV reservoir.
