KLH acts as a carrier and immunogenic stimulus, while sargramostim recruits dendritic cells and enhances antigen presentation. The product is used as an individualized immunotherapy primarily for follicular lymphoma and multiple myeloma, where the manufacturing process involves isolating the tumor immunoglobulin idiotype from the patient, synthesizing the KLH conjugate, and co-administering with GM-CSF. Mechanistically, the vaccine induces humoral and cellular anti-tumor responses, while GM-CSF boosts local dendritic cell recruitment and antigen processing. This approach has been studied in several clinical trials, with evidence of improved disease-free survival and idiotype-specific immune responses in some subgroups.
