Overview

Autologous mesenchymal stem cells (MSCs) are a cell-based therapy developed by Rigshospitalet (specifically the Cardiology Stem Cell Centre) for the treatment of severe chronic myocardial ischemia and coronary heart disease. The therapeutic process involves harvesting bone marrow from the patient, isolating and expanding the mesenchymal stromal cells in a controlled laboratory environment, and subsequently delivering them back to the patient. In the context of myocardial ischemia, these cells are typically administered via intramyocardial injection, often guided by electromechanical mapping systems like NOGA. The primary mechanism of action is believed to be paracrine signaling; the transplanted MSCs secrete a variety of growth factors and cytokines, including vascular endothelial growth factor (VEGF) and hepatocyte growth factor (HGF). these factors stimulate vasculogenesis and angiogenesis, reduce myocardial inflammation, and promote the repair of ischemic tissue, thereby improving myocardial perfusion and clinical symptoms in patients who have no further conventional revascularization options.
