Overview

Autologous micro-fragmented adipose tissue (MFAT) is an investigational biologic therapy developed by Copenhagen University Hospital Hvidovre (Hvidovre University Hospital) for the treatment of knee osteoarthritis. The procedure involves harvesting the patient's own adipose tissue, which is then mechanically processed using the Lipogems system to create a micro-fragmented graft. This processing preserves the stromal vascular fraction (SVF), which contains adipose-derived mesenchymal stem cells (ASCs), pericytes, and various growth factors within their natural niche. The therapy is intended to provide anti-inflammatory and potentially regenerative effects when injected intra-articularly. A multicenter randomized controlled trial conducted by Hvidovre University Hospital and Zealand University Hospital evaluated the efficacy of a single 10-ml injection in patients with Kellgren-Lawrence grades 2-3 knee osteoarthritis. Results published in 2025 indicated that the treatment was not superior to a saline placebo in improving patient-reported outcomes at a 2-year follow-up.
