Overview

Autologous peripheral blood stem cell transplantation (ASCT) is a therapeutic procedure and cell therapy protocol used to treat relapsed or refractory AIDS-related lymphoma. The process involves mobilizing a patient's own hematopoietic stem cells from the peripheral blood, typically using growth factors like filgrastim, followed by collection via apheresis and cryopreservation. The patient then undergoes a high-dose conditioning chemotherapy regimen, such as the combination of busulfan and cyclophosphamide, which is designed to eradicate malignant lymphoma cells but also results in bone marrow ablation. The stored stem cells are subsequently re-infused into the patient to rescue and reconstitute the hematopoietic system. The AIDS Malignancy Consortium (AMC) conducted pilot studies (e.g., AMC-020) to establish the safety and feasibility of this intensive approach in HIV-positive patients, demonstrating that it can be performed successfully when HIV viral replication is controlled with antiretroviral therapy.
