Overview

Autologous platelet lysate (PL) is an autologous peripheral blood-derived orthobiologic (APBO) investigated for the treatment of knee osteoarthritis. It is produced by the controlled lysis of platelets—often through freeze-thaw cycles—to release a high concentration of growth factors and bioactive molecules, including PDGF, TGF-β, VEGF, and IGF-1, from alpha-granules. Unlike platelet-rich plasma (PRP), which contains intact platelets, PL provides these factors in a readily available form to stimulate chondrocyte proliferation, promote extracellular matrix synthesis, and exert anti-inflammatory effects within the joint. National Taiwan University Hospital has evaluated this therapy in a Phase 4 randomized, placebo-controlled trial (NCT03734900) to compare its efficacy against PRP and saline in patients with Kellgren-Lawrence grade I-III knee osteoarthritis, aiming to reduce pain and improve joint function.
