Overview

Autologous room temperature stored platelet transfusion involves the collection and storage of a patient's own platelets at approximately 22°C for a period of up to 7 days prior to re-infusion. Developed by Bloodworks (formerly Bloodworks Northwest), this intervention is being studied in the Cold Stored Platelets for Aspirin Reversal (RASP) trial as a comparator to cold-stored platelets (4°C). The primary therapeutic goal in this context is the reversal of platelet aggregation inhibition caused by aspirin. Aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) in platelets, preventing the formation of thromboxane A2 and subsequent aggregation. By transfusing autologous platelets that have not been exposed to aspirin, functional COX-1 and aggregation capacity are restored to the patient's blood, potentially reducing bleeding risks in surgical or trauma settings.
