Autologous stem and progenitor cells are utilized in regenerative endodontic procedures (REP) to treat immature permanent teeth with pulp necrosis and apical periodontitis. In the protocol developed by Universitaire Ziekenhuizen KU Leuven, these cells—primarily stem cells from the apical papilla (SCAP)—are recruited into the root canal space following disinfection. The procedure often employs autologous leucocyte and platelet-rich fibrin (L-PRF) as a bioactive scaffold. L-PRF provides a three-dimensional fibrin matrix and a sustained release of growth factors (such as PDGF, TGF-β, and VEGF) that promote cell migration, proliferation, and differentiation into odontoblast-like cells, ultimately facilitating root development and periapical healing.
