Overview

**Autologous thrombin** is a biologically active enzyme (serine protease) prepared intraoperatively from a patient's own blood, typically using a point-of-care device. It is utilized as a topical hemostatic agent and as an activator for platelet-rich plasma (PRP) and platelet gels to enhance wound healing. Its primary mechanism is the conversion of soluble fibrinogen to fibrin, thereby promoting clot formation and stabilization at surgical or wound sites. Unlike bovine or pooled human thrombin, it avoids immunogenic reactions and transmission risks since it is derived from autologous (self) tissue. Commercially available processing systems (e.g., Thrombinator by Arthrex) or similar devices produce autologous thrombin from whole blood or platelet-poor plasma by activating the coagulation cascade and collecting the active thrombin-rich serum for immediate clinical application[1][3].
