Autologous TRAIL-secreting adipose perivascular stromal cells is an investigational cell and gene therapy developed by Eir Biotherapies for the treatment of pancreatic adenocarcinoma. The therapy involves harvesting perivascular stromal cells (APSCs) from a patient's own adipose tissue and genetically modifying them to express and secrete Tumor Necrosis Factor-Related Apoptosis-Inducing Ligand (TRAIL). These engineered cells are designed to leverage the natural tropism of stromal cells for tumor microenvironments, allowing them to home to tumor sites and provide localized delivery of TRAIL. Once at the tumor, the secreted TRAIL selectively induces apoptosis in malignant cells by activating death receptors DR4 and DR5, while sparing healthy tissues. This approach aims to overcome the therapeutic limitations of recombinant TRAIL, such as its short systemic half-life and poor penetration into dense pancreatic tumor stroma. The program has been granted Orphan Drug Designation by both the FDA and the EMA.
