This combination immunotherapy regimen consists of autologous tumor-infiltrating lymphocytes (TILs) and low-dose interleukin-2 (IL-2), specifically studied by the University Health Network (UHN) in Toronto for the treatment of metastatic melanoma. The therapeutic process begins with the isolation of naturally occurring T cells from a patient's resected tumor specimen. these cells, which have already demonstrated the ability to recognize tumor-associated antigens, are expanded to large numbers ex vivo. Before the TIL infusion, patients undergo a non-myeloablative lymphodepleting preparative regimen, typically using cyclophosphamide and fludarabine, to eliminate regulatory T cells and competing lymphocytes. Following the infusion of the expanded TILs, low-dose IL-2 is administered to promote the in vivo survival, proliferation, and anti-tumor effector functions of the transferred lymphocytes. This approach leverages the patient's own immune system to target and destroy melanoma cells throughout the body.
