Overview

Autologous WT1-specific T cells are a form of adoptive cellular immunotherapy designed to target the Wilms tumor protein 1 (WT1), a zinc-finger transcription factor that is highly overexpressed in various hematological malignancies and solid tumors while being minimally expressed in normal adult tissues. The production process involves harvesting a patient's own peripheral blood mononuclear cells (PBMCs), which are then stimulated *ex vivo* with WT1 peptide-pulsed antigen-presenting cells—often autologous B-lymphoblastoid cell lines (BLCL) or dendritic cells—to enrich and expand T cell populations that naturally recognize WT1 epitopes. These expanded cytotoxic T lymphocytes (CTLs) are reinfused into the patient, frequently following lymphodepleting conditioning chemotherapy to facilitate their survival and expansion. Once in the body, these cells recognize WT1-derived peptides presented by HLA molecules on tumor cells, leading to targeted cell lysis through the release of perforin and granzymes.
